Remember Someone Special

Remember the life of a loved one by making a special in-memoriam gift to Baker IDI Heart and Diabetes Institute. Your thoughtful donation will ensure the memory of your loved one will live on, but it also provides hope to so many others who are affected by heart disease and diabetes.

Appropriately designed in memoriam donation envelopes are available on request, and with a little notice these can be made available at a funeral, wake, church or other commemorative service where people may choose to make a donation.

To request donation envelopes contact our fundraising department on 1800 827 040.

If you are pressed for time, download our specially designed In Memoriam Donation Form

Create a Tribute Page

An alternative way to give in memory is to create a special, personalised webpage, where everyone can donate and leave individual tribute messages. In just a few simple steps, you will be able to fundraise in your loved ones honour and celebrate their life in a meaningful way.

Donations over $2 are tax deductible and a tax receipt will be issued for each gift made.
