Our services are available to everyone. We respect each client’s right to receive high quality healthcare that is safe and appropriate for them.

What are your rights and responsibilities as a client of Baker IDI Education Services and what to expect from our service and staff? 


To book or reschedule an appointment, please telephone reception (03) 8532 1800.

Appointments are available Monday to Friday, 8.45am–4.15pm.
Please allow 60 minutes for initial appointments and 30 minutes for review appointments.

Referrals from health professionals can be faxed to (03) 8532 1899.

Health Professionals, download a referral form. Self referrals are also welcome.


Most Education Service appointments are subsidised by Department of Health funding.

Appointment fees start at $10 for people with a health care or pension card and increase to $95 for an initial appointment ($45 for review appointments) according to a person’s income level.

Gestational Diabetes Service includes appointments with a Diabetes Educator and Dietitian and weekly email contact for the duration of the pregnancy. Fees start at $30 per appointment for people with a health care or pension card and increase to $110 for according to a person’s income level.

Commencement of Insulin Therapy includes an appointment with a Diabetes Educator and follow up telephone or email contact. Fees start at $30 for people with a health care or pension card and increase to $95 for according to a person’s income level.

Private Fee Services include appointments for insulin pump therapy and continuous glucose monitoring.

Team Care Arrangement plans coordinated by a general practitioner with allocated visits to allied health practitioners are accepted. Allocated visits are bulk billed. An out of pocket fee applies to initial private fee service appointments.

Cancellation Fee will be applied for cancellations with less than 24 hours notice or for non attendance at a scheduled appointment.


AusLANLanguages Other than English & Australian Sign Language (AusLAN)

A professional interpreter in your preferred language can be arranged for appointments if advance notice is provided. This service is free to clients attending Education Service appointments.

Please advise if you, a family member or client requires an interpreter at the time of booking.

Education Services are situated within the Baker IDI Specialist Diabetes Clinic, located at:

Public transport

Bus routes 216, 219 and 220 all stop at the corner of Commercial and Punt Roads.

Tram number 72 stops at the corner of Commercial and Punt Roads.
Tram numbers 3, 5, 6, 16, 64 and 67 travel along St Kilda Road. Alight at the corner of St Kilda Road and Commercial Road, and walk approximately 600 meters on Commercial Road to reach the clinic.

Train on the Sandringham line, alight at Prahran station and walk north along Charles Street to Commercial Rd. Turn left onto Commercial Road and continue toward Punt Rd (approximate walking distance 900 metres). Pakenham line, alight at Toorak station and catch Tram Number 72 to the corner of Commercial Road and Punt Road.

Car parking

Underground parking is available on-site, however availability can be limited at peak times (fees apply).

Two hour street parking is available in some nearby streets.

Casual parking is available at Prahran Market, a 15 minute walk along Commercial Road (fees apply).
